To have us get started, please follow the following steps:

1. Complete and send the Request for mediation by Zoom form to us.

2. Read, sign, and return the completed Mediation Agreement. Pay your portion of the $800 deposit. It should be divided equally between or among the parties

3. Read, sign, and return the completed Mediation Disclosure Notification for Party. This must be done by each party to the mediation.

4. Obtain a signed Mediation Disclosure Notification and Acknowledgment for Non-Party form for each non-party that you expect to participate as a witness or who will otherwise attend the mediation at your request. The other party will be required to do the same.

5. Review the FAQ - Mediations to educate yourself about the mediation process.

6. Review the Zoom Mediation Procedures and the Glossary of Mediation Terms.

7. Call Michael Chulak with any questions. You may call as often as needed. We want you to be fully informed.
